Momentum’s Psychiatric Rehabilitative Program (Adult & Minor)

Individual and group therapeutic sessions are provided by trained professionals at Momentum’s office, the participant’s home, or other appropriate community locations. Both in- office and in-home programs are person-centered, tailored to their unique needs and capabilities, with a particular focus on helping individuals and their families overcome challenges.

After gathering information from the referring mental health professional, conducting a thorough evaluation, and meeting with the individual and their family, specific rehabilitation goals are set to address identified needs. Staff members from PRP regularly assess the development of each participant and report back to the referring mental health provider, guardian, and any other mental health providers involved in their care.

Psychiatric Rehabilitative Assessment

In order to determine the individual’s strengths, weaknesses, and mental rehabilitation requirements, a functional skills assessment is conducted. This assessment occurs in a face- to-face meeting between the participant and if needed, a member of their family, guardian, or caretaker upon admission to the program.

Individual and Group Therapy

The primary objective of both individual and group rehabilitation activities is to mitigate functional behavioral impairments while fostering the growth and restoration of age- appropriate skills across a variety of areas. These areas encompass, but are not limited to:
